College Preparation
Students, if you need a college recommendation letter from the SITES teachers, please use this form.
When submitting a college recommendation letter request, please keep the following in mind:
- All student information on this college/scholarship application information must either be typed and printed, or submitted using this form
- If you are going to type and print your request (not using this form), please make sure that it is in neat, clean condition
- If the recommendation letter is to be forwarded directly to the college(s), please be sure to turn in an addressed STAMPED envelope
- It is expected that a minimum of one week's time be given to the teacher(s) to be able to complete your recommendation. After one week, please follow-up on its completion
- Common courtesy would suggest that you ultimately follow-up with information regarding the teacher(s)' efforts, i.e., whether you were accepted to the university, scholarship offerings, etc.
- Please try to have your completed recommendation copied in guidance or the main office before mailing, so it can be used for future recommendation requests, if needed
