This form must be filled out for all computer equipment taken for use off campus. This form must be printed out in addition to being submitted online. Send the signed printed copy of the completed form to your building media specialist.

In borrowing the above equipment I agree to accept full responsibility for its return in the condition it was loaned. I realize that the district maintains a $1,000 deductible insurance policy and that such deductible is my responsibility in case of theft while in my care. I agree to pay for any damage either directly or indirectly to the equipment in my care, including reinstallation or repair of software, determined by an examination of it performed by the maintenance provider upon its return.
